Our staff do all they can to ensure patients can leave hospital in a timely and supported way, but we know that discharges can be delayed for a number of complex reasons, often in relation to further care the patient will need after they leave hospital.
Whilst we arrange the patient's ongoing care package, we want to ask relatives, friends and carers to support us by collecting their loved ones as soon as they are well enough to leave hospital and to provide some basic care in the meantime.
This can include arranging regular help at home to help the patient get washed, fed and dressed, collecting their prescriptions and making any other necessary adaptations to your home.
We are currently extremely busy with very high numbers of patients attending hospital and by supporting your loved one's recovery, you will also be helping us to free up our beds for other patients in need of hospital care.
For more information on how you can help, please see our Leaving Hospital leaflet.
If you have a disability, sensory loss, impairment, or if English is not your first language, we can help you access information about our services.
It's important for us to know when we've provided a good service or if something needs to improve.
