New grant scheme is available to support discharges from hospital

A new grant scheme to support patients get home as soon as possible has recently launched at Great Western Hospitals NHS Foundation Trust. 

The Discharge Grant Scheme will provide a grant to support individuals with small packages of care and assistance that may help them return home.  For example, this could include additional help with cleaning or someone to help with cooking and shopping. The support of informal care to the patient could be provided by a family member or from another individual or company, depending on the need. The funding will support the individual for 4 weeks post discharge.

Patients that are clinically ready for discharge from hospital will be assessed to ensure they meet specific criteria and will then be able to receive the grant.

The local NHS Clinical Commissioning Group are working with CHS Healthcare to deliver the scheme.
