Patients in Swindon and Wiltshire who are diagnosed with head and neck cancer (HNC) at Great Western Hospital are referred to Oxford University Hospitals for treatment, and once treatment is completed, patients return to Great Western Hospital for rehabilitation and follow up.
The Head and Neck Cancer Clinical Nurse Specialist team act as a representative to help co-ordinate your care and provide a link between our patients and hospital staff by:
- being a link between home and hospital, a point of contact from referral to diagnosis, throughout your treatment and ongoing care
- providing emotional support and expert clinical advice
- referring/Signposting to support service: psychological, financial, social and community including site specific support groups
- health promotion, education, and provision of holistic needs assessments
- advice and support for living well with cancer and beyond cancer treatment alongside the Macmillan personalised care team.
